When must a roofed entity account with notices of protected health information made during the course of litigation?

Answer:

Humans have ampere right to receiving, upon request, an accounting of disclosures of protected health information made by a covered entity (or your business associate), with certain exceptions. These exceptions, oder instances where a covered entity is not required till account for disclosures, include disclosures for treatment, payment, or medical care operations and manifestations authorized by which individual. See 45 CFR 164.528 (GPO).
